Preclinical Testing of Total Hip Stems

چکیده
The long-term fixation endurance of noncemented hip stems in total hip arthroplasty is subject to incompatible design goals. To reduce stress shielding and periprosthetic bone loss, proximal fixation and load transfer are indicated. However, to prevent interface motion and promote interface-bonding security, fixation preferably should be maximized over the entire stem surface. In this study, the authors questioned whether hydroxyapatite coatings could be applied in patterns that reduce bone resorption, While maintaining safe interface stress levels. For that purpose, strain-adaptive bone-remodeling theory was applied in 3-dimensional finite element models, to simulate the long-term postoperative bone resorption process. During the process, the adaptation of interface stresses was monitored, and its effects on interface failure probability evaluated. This analysis was done for a fully coated stem, a 'h proximally coated stem, a smooth uncoated, press-fitted stem, and a stem with 5 proximal patches of circumferential stripes. The uncoated stem reduced bone loss dramatically, but promoted interface motions and dis-

Total Hip Replacement Revision in a Single Brand Small Cementless Stem – Our Experience after the Findings of the National Joint Registry
Background: Cementless total hip replacement is the common THR performed in England, Wales, Northern Irelandand the Isle of Man. The Corail stem is the most popular cementless implant and has a ODEP 10A rating. Review ofits performance in the registry identified an increase rate of revision amongst the smaller stem sizes. However, claritywas not provided on the explanation for this finding. We ...
متن کاملA novel test method for evaluation of the abrasive wear behaviour of total hip stems at the interface between implant surface and bone cement.
After total hip replacement, some cemented titanium stems show above-average early loosening rates. Increased release of wear particles and resulting reaction of the peri-prosthetic tissue were considered responsible. The objective was to develop a test method for analysing the abrasive wear behaviour of cemented stems and for generating wear particles at the interface with the bone cement. By ...
متن کاملBone density study of the proximal femur after hip arthroplasty with porous-coated implants
Progressive loss of bone mineral density around the femora! component of total hip replacement continues to pose a threat to long term prosthetic survival. A linear study was undertaken to measure bone mineral density on a monthly basis following total hip arthroplasty in 11 male patients. The opposite femur was used as the control measurement. Bone mineral density was unchanged at two mo...
متن کاملHip contact forces in asymptomatic total hip replacement patients differ from normal healthy individuals: Implications for preclinical testing.
BACKGROUND Preclinical durability testing of hip replacement implants is standardised by ISO-14242-1 (2002) which is based on historical inverse dynamics analysis using data obtained from a small sample of normal healthy individuals. It has not been established whether loading cycles derived from normal healthy individuals are representative of loading cycles occurring in patients following tot...
متن کاملThe relationship between stress shielding and bone resorption around total hip stems and the effects of flexible materials.
Bone resorption around hip stems is a disturbing phenomenon, although its clinical significance and its eventual effects on replacement longevity are as yet uncertain. The relationship between implant flexibility and the extent of bone loss, frequently established in clinical patient series and animal experiments, does suggest that the changes in bone morphology are an effect of stress shieldin...
